you get hurt when you run, first thing you should do is hunt.
you can usually wait longer than you think.

Are the enemies shooting at you while you run? I have no idea what's damaging your pig while it runs to or from combat.

You actually are hurt, and can't run without losing life! It's barely explained in the tutorial, however, i know, i know, it's a wall of text. It's important for the balance of the game, otherwise you could just run away from everything.
(man, isn't the fact that running away from everything is really hard such a good metaphor though? hahahah)
